Phase 01

Pre-Consult

  1. Patient initiates contact

    Patient books an appointment via the patient app, clinic website, or receptionist. The system generates a pre-consult intake link automatically.

  2. AI-guided symptom collection

    The patient completes a structured, adaptive symptom questionnaire. Questions branch based on responses to collect clinically relevant information efficiently.

  3. Triage and risk scoring

    The AI analyses intake responses and flags high-priority cases, suggests appointment urgency, and generates a pre-consult summary for the clinician.

  4. Clinician briefing

    Before the appointment begins, the clinician receives a structured summary of the patient's presenting complaint, relevant history, and any flagged risk indicators.

Phase 02

Consultation

  1. Consultation begins — transcription activates

    Once the clinician opens the patient record, ambient transcription begins automatically. The clinician and patient speak naturally; no manual note-taking is required.

  2. Real-time clinical assistance

    The AI Assist panel surfaces relevant clinical guidelines, drug interactions, and suggested diagnostic paths based on the conversation content in real time.

  3. Telehealth (where applicable)

    For remote consultations, the integrated video session runs within the same interface. Documentation and AI assist functions work identically in telehealth mode.

  4. Note structure generated

    At consultation end, the AI produces a structured SOAP note — Chief Complaint, History, Examination findings, Assessment, and Plan — for clinician review and sign-off.

Phase 03

Post-Consult

  1. Note review and sign-off

    The clinician reviews the AI-generated note, makes any amendments, and signs off within the platform. Average review time is under 90 seconds.

  2. Automated outputs

    The platform automatically generates referral letters, pathology requests, and patient instructions based on the consultation note and clinician selections.

  3. Care plan creation

    Where indicated, a structured care plan is generated using guideline-based templates and personalised to the patient's consultation outcome.

  4. Follow-up scheduling

    Follow-up reminders and appointment suggestions are generated automatically based on the care plan timeline and sent to the patient via the app.

Phase 04

Continuous Care

  1. Longitudinal record update

    All consultation data, care plans, and outcomes are automatically appended to the patient's longitudinal health record, accessible to all authorised providers.

  2. Immunisation and preventive care tracking

    The platform monitors immunisation schedules, health screening intervals, and chronic disease management checkpoints, triggering reminders as needed.

  3. Multi-provider coordination

    For patients with complex or shared care, the Synchronised Patient Care module coordinates care tasks, shared notes, and outstanding actions across providers.

  4. Population health monitoring

    Practice-level dashboards identify cohorts requiring follow-up, overdue screening, or chronic disease review, enabling proactive care management at scale.
